Our clinic provides services such as the release of medical records and completion of insurance claim forms. However, the release of medical information is subjected to approval by the clinic.
How do I request for a Medical Report?
The patient's written consent and stated purpose for the release of medical information is required for the clinic to proceed with the authorisation of release.
Step 1: Patient has to fill up the online Authorisation to Release Medical Report form and prepare the necessary documents as required.
For patients below 21 years old, the consent of the parent or legal guardian is required. Please bring along patient’s birth certificate and parent / legal guardian NRIC.
For deceased patients, patient’s death certificate and other documents e.g. birth certificate, marriage certificate or any proof of your relationship to the deceased is required.
For mentally incapacitated patients, the written consent of his/her authorised representative or all his/her immediate next of kin must be brought.
The Medical Report may take up to 3 weeks to process from time of approval and payment.
Step 2: Payment for the services will be required before the medical information is processed for release. Payment has to be made at our clinic.
Please bring along the original copy of the documents to the clinic while you make payment.
If the fees are below S$50, please pay by cash; otherwise we accept Visa, MasterCard or American Express credit cards. Cheques made may be crossed and made payable to “Dr. Jonathan Lee Specialist Surgery”.

Completion of an insurance form by our staff — S$50
Brief information containing diagnosis, name of procedure, ICD9 / ICD10 codes and visit dates (No signature by Doctor required).
Completion of a detailed insurance form by Dr. Lee — starts from S$150
Information required may consist of diagnosis, name of procedure, and details of medical condition or treatment given.
Specialist Medical Report — starts from S$600
This report is prepared by the patient's specialist in response to requests that require a professional opinion with regards to the patient's prognosis and disabilities. It is based on an actual assessment of the patient and may involve a review at the Specialist Outpatient Clinic.
Duplicate Copy of Investigation Results — S$15 per copy
Selective duplicate copies of investigation results such as blood test results, lab results, investigation reports (ECG, Electrophysiology, EMGS), X-ray and other radiological reports can be made available.
Certified True Copy of Medical Report — S$15 per copy
A duplicate copy of medical report that was previously applied before.
Overseas Handling Fee — S$10
For medical reports that needs to be sent out of Singapore, an overseas handling fee applies.
Reprint of Medical Certificates Issued — S$10 per copy
A duplicate copy of electronic medical certificate issued by the wards and clinics.
